Community Health Improvement Plans
The Community Health Improvement Plan (CHIP) Project is a collaborative initiative between Barnes and the Indiana University Center for Rural Engagement (CRE), aimed at enhancing health outcomes in rural communities. The project leverages community health assessments—or develops new ones when necessary—to identify key health priorities within local populations. By gathering and analyzing data on the most pressing health issues, the CHIP Project empowers local groups and coalitions to design and implement targeted interventions.
This partnership emphasizes the importance of strategic collaboration, bringing together community leaders, healthcare providers, and other stakeholders to form robust networks that can effectively address health disparities. The initiative focuses on building local capacity, promoting evidence-based practices, and fostering sustainable health improvements.
